Patients must have a valid referral from their GP or other treating specialist to be able to claim a rebate from Medicare, DVA or Private Health insurance.
GP referral last for 12 months from the date of first service, and Specialist referrals last for 3 months.
Your Private Health insurance will only contribute to the cost of your in hospital treatment, not consultations in the rooms.
What to bring to your appointment?
- Medicare card
- Centrelink pension card (if applicable)
- Private Health Insurance card (if applicable)
- Veteran Affairs card (if applicable)
- List of current medications from your GP or pharmacist with name of medication, dosage and how often
- Pathology/radiology
You will be required to complete a Patient information and consent form when you visit the practice for the first time. This form can be printed from the Forms tap at the top of the page, complete and bring with you to the appointment if you wish to save time at your appointment.
We appreciate that many people travel long distance for an appointment and we will always use our best endeavours to streamline your visit and minimise your waiting time.
It is our practice policy to receive patient referral prior to making appointment to allow the practice to triage appointments.
Referrals can be send to the rooms via Post / Fax / email.
Unfortunately not every patient will be seen as quickly as they would like. We have a cancellation list and are regularly checking this to offer earlier dates where possible.
